1007 Audit of Time to Surgery for Decompressive Craniectomy in Ischaemic Middle Cerebral Artery Stroke

Abstract Aim Early decompressive craniectomy (DC) in patients with ischaemic middle cerebral artery (MCA) stroke is closely related to post-surgical outcomes terms of mortality, morbidity, and functionality. This audit compares the time surgery for MCA Hospital Sultanah Aminah Johor Bahru (HSAJB), Malaysia, recommended 48-hour timeframe by National Institute Health Care Excellence. Method included who underwent DC HSAJB from 2017 2019. Time symptoms onset was obtained retrospectively Neurosurgery Department patient records. Data analysed compared functional within 18 months post-surgery, where Modified Rankin Scale (mRS) 0–3 represents favourable while mRS 4–6 shows severe disability or mortality. Ethics approval not required. Results 16 out 38 met inclusion criteria had clear documentation (n=16). 75% (n=12) 48 hours. Of these patients, only 25% (n=3) achieved post-surgery. Nevertheless, hours demonstrated relatively positive after hours, no (n=0) 0–3. Conclusion Majority timeframe. Despite low probability achieving good showed better chance survival less disability. It aim all outcomes.
